Trials of acupuncture for shoulder pain have mostly been small, and they mix together different problems such as rotator cuff pain, impingement and frozen shoulder. Some report less pain and better movement in the weeks after treatment compared with no treatment. Comparisons with sham acupuncture show smaller differences, and longer-term benefit is uncertain. In practical terms, acupuncture is a reasonable thing to try if pain is stopping you from doing your exercises or from sleeping, as long as you keep the exercise plan at the centre and agree a review point at the start.

What NICE and the NHS say

  • Shoulder pain (NICE CKS). For rotator cuff problems, NICE clinical knowledge summaries advise pain relief, adjusting activities that make pain worse such as reaching overhead, and a course of physiotherapy, usually 6 weeks, which can continue for another 6 weeks if it is helping. A steroid injection can be considered alongside physiotherapy.
  • Frozen shoulder (NICE CKS). Pain relief, keeping the arm moving within comfort, early physiotherapy and, for some people, an early steroid injection into the joint. The summary notes that physiotherapy can include acupuncture performed by the physiotherapist, but it does not recommend acupuncture as a treatment in its own right.
  • Chronic primary pain (NICE NG193). A single course of acupuncture can be considered for pain lasting more than three months with no clear underlying cause. Shoulder pain with a clear cause, such as a tendon problem or frozen shoulder, does not fall into this group.

The NHS advises that you usually need to keep up gentle movement, exercises and pain relief for around two weeks before shoulder pain starts to ease, and that full recovery can take six months or longer.

Acupuncture for rotator cuff pain and impingement

Rotator cuff pain, often called impingement, is one of the most common types of shoulder pain. It usually hurts when you lift the arm out to the side or reach overhead, and it often aches at night. The treatment with the strongest support is a graded exercise programme that strengthens the rotator cuff and the muscles around the shoulder blade.

Acupuncture for frozen shoulder

Frozen shoulder causes pain, which is often worse at night, followed by stiffness that makes it hard to move the arm in any direction. The NHS explains it can take months or years to get better, but the pain and stiffness usually go away eventually. It is more common in people with diabetes.

What happens at an acupuncture session for shoulder pain

  1. Assessment. Your practitioner asks how the pain started, which movements hurt, how you are sleeping and about your general health, and checks for signs that need your GP first.
  2. Needling. Fine, sterile, single-use needles are placed around the shoulder, across the top of the shoulder and shoulder blade, and at points on the arm and hand. They stay in for around 20 to 30 minutes. Most people feel a dull ache or tingle rather than sharp pain.
  3. Added techniques. Cupping or massage may be used on tight muscles across the upper back.
  4. Movement advice. You leave with simple exercises to keep the shoulder moving.

Most people start with one session a week. If nothing has changed by then, we will suggest stopping or switching the focus to physiotherapy rather than carrying on by default.

  • Your arm or shoulder is hot or cold to touch, or you have severe pain in both shoulders
  • You feel feverish or generally unwell with shoulder pain

See your GP if shoulder pain is getting worse or has not improved after two weeks, or it is very difficult to move your arm.
