Back pain is the most common reason people try acupuncture, and also one of the most debated. Some people swear by it; others find it does little. The official guidance has also changed over the years. This guide gives you an honest picture of what acupuncture for back pain can and cannot do, so you can decide whether it is right for you.

NICE recommends acupuncture as an option for chronic primary pain, meaning pain lasting more than three months without a clear underlying cause. Does acupuncture help back pain? What the evidence shows

Research reviews of acupuncture for chronic low back pain find that it reduces pain and improves function compared with no treatment, at least in the short term. Compared with sham acupuncture, the difference is smaller, which suggests part of the benefit comes from the treatment experience as a whole. When acupuncture is added to exercise or usual care, many people report better results than with usual care alone.

In practical terms, acupuncture is a reasonable option if your back pain has lasted months, if other treatments have not helped enough, or if pain is stopping you from doing the exercises that will help you most. It is less useful as a stand-alone treatment for a new episode of back pain, which usually settles within six weeks with activity and simple pain relief.

What NICE says

  • Low back pain and sciatica (NICE NG59). NICE does not recommend acupuncture for managing low back pain with or without sciatica. It recommends exercise first, with manual therapy only as part of a package that includes exercise.
  • Chronic primary pain (NICE NG193). NICE recommends considering a single course of acupuncture for adults with chronic primary pain, which is pain lasting more than three months that is not better explained by another condition. Some people with long-term back pain fall into this group.

  • It changes pain signalling. Needles stimulate nerves that trigger endorphin release and dampen pain messages in the spinal cord and brain.
  • It eases muscle guarding. Tight muscles around the spine and hips often relax during treatment, which can make movement easier.
  • It opens a window for exercise. Many people find they can do their rehabilitation exercises more comfortably after sessions, which is where lasting improvement comes from.

Acupuncture for back pain and sciatica

For sciatica, where a nerve in the lower back is irritated, the main treatments are staying active, exercise and, if needed, physiotherapy or medication from your GP. Acupuncture for back pain in pregnancy

Which acupuncture points are used for back pain?

Your practitioner chooses points based on where your pain is and what drives it. Points along the lower back and buttock are commonly used, along with distant points behind the knee (BL40) and on the hand (Yaotongxue). Electro-acupuncture, which adds a gentle current, is sometimes used for stubborn pain. Cupping or Tui Na massage may be added to work the surrounding muscle. How many sessions do you need?

For chronic back pain, most people try one session a week for four to six weeks, then review. NICE describes a single course for chronic primary pain. If there is no meaningful change after five or six sessions, it is better to focus on other approaches.
