Losing a lot of weight is a huge achievement, whether it came from changing how you eat, from exercise, from weight-loss injections such as Mounjaro or Wegovy, or from surgery. For many people, though, the finish line brings a new worry: skin that feels soft, loose or crepey, most often on the stomach, arms, thighs and neck. This guide explains why loose skin happens, whether it goes away on its own, how to reduce the risk while you are losing weight, and what can tighten it afterwards, with and without surgery.

Firmer skin with no needles and no downtime, at our Chiswick clinic. Your first LPG Endermologie visit is a 60-minute Discovery Session for £99, including a skin assessment and a full treatment. Book online or call 020 4515 0000.

Quick answer

Loose skin after weight loss happens when skin that has been stretched for a long time cannot shrink back fully over a smaller body. You can lower the risk by losing weight steadily, eating enough protein, doing strength training, staying hydrated, protecting your skin from the sun and not smoking. Mild loose skin often improves on its own over the following months. To tighten it further, strength training builds shape underneath, and non-surgical treatments that stimulate collagen, such as LPG Endermologie, radiofrequency or ultrasound, can firm mild to moderate looseness. Large folds of overhanging skin usually need surgery.

Why does skin go loose after weight loss?

Skin gets its strength from collagen and its stretch and recoil from elastin. When the body grows, skin grows with it. When weight is carried for years, those fibres are stretched for a long time and some are damaged, so the skin loses part of its ability to spring back. When the weight comes off, the skin is left with more surface than the body underneath needs.

How loose your skin becomes depends on several things:

  • How much weight you lose. The larger the loss, the more spare skin is left.
  • How long you carried the weight. Skin that was stretched for many years recoils less well.
  • How quickly you lose it. Fast loss gives the skin less time to adapt, which is one reason loose skin is common after bariatric surgery and weight-loss injections.
  • Age. From our mid-twenties the skin makes less collagen and elastin each year.
  • Genes. Some people simply have more elastic skin than others.
  • Sun damage and smoking. Both break down collagen.
  • Pregnancy. Stretching of the stomach in pregnancy adds to looseness there.

Does loose skin go away on its own?

Sometimes, at least partly. Skin keeps remodelling for many months after weight loss, so mild looseness often improves noticeably once your weight has been stable for a while, especially if you are younger, lost a moderate amount of weight and kept your muscle. Large amounts of loose skin after major weight loss rarely disappear completely without help. If skin hangs in folds, it will not go back on its own.

How to prevent loose skin when losing weight

You cannot fully control how your skin responds, but these habits give it the best chance to keep up:

  • Lose weight steadily. The NHS suggests aiming for around 0.5 to 1 kg a week. If you are on weight-loss medication, talk to your prescriber about the dose and pace that suit you.
  • Eat protein at every meal. Protein protects muscle and gives the skin the building blocks it needs to make collagen, even when your appetite is small.
  • Do strength training two or three times a week. Muscle fills out the space under the skin, so keeping it makes loose skin less visible.
  • Drink enough water. Well-hydrated skin looks smoother and plumper.
  • Use sunscreen and do not smoke. Both protect the collagen you have.
  • Start supporting the skin early. Collagen-stimulating treatment during weight loss, rather than only afterwards, can help the skin keep pace.

How to tighten loose skin after weight loss

The right option depends on how much loose skin you have and where it is. Most people combine more than one approach.

At home

  • Strength training. Building muscle will not shrink skin, but it fills it out and improves shape, particularly on the arms, legs and buttocks.
  • A good skincare routine. Retinoids and vitamin C can support collagen and improve skin texture over months, but their effect on looseness is modest. Moisturisers make skin look and feel better without tightening it.
  • Collagen supplements. Some small studies, many of them funded by manufacturers, suggest slight improvements in skin hydration and elasticity. There is no good evidence that they tighten noticeably loose skin.

Non-surgical treatments

  • Mechanical skin stimulation. LPG Endermologie uses gentle suction and motorised rollers to stimulate the skin's own production of collagen, elastin and hyaluronic acid. There is no heat, no needles and no recovery time, and it treats large areas such as the stomach and thighs as well as the face and neck.
  • Radiofrequency. Heats the deeper layers of the skin to trigger new collagen.
  • Focused ultrasound. Delivers heat to deeper tissue, most often on the face and neck.

Non-surgical treatments work best for mild to moderate looseness and crepey skin. They firm and smooth the skin over a course of sessions rather than removing it.

Surgery

Skin removal surgery, such as a tummy tuck (abdominoplasty), arm lift, thigh lift or body lift, is the only way to remove large amounts of excess skin. It leaves scars and needs several weeks of recovery, and surgeons usually ask for your weight to have been stable for some months first. It is rarely available on the NHS unless excess skin is causing medical problems.

Loose skin on the stomach

The stomach is where most people notice loose skin first, because it often stores the most weight and may also have been stretched by pregnancy. Mild looseness and crepey texture on the stomach often respond well to a course of LPG Endermologie alongside core and strength training. A fold of skin that hangs over the waistband, sometimes called an apron or pannus, usually needs a surgical opinion. If your stomach still bulges after pregnancy, ask your GP or a women's health physiotherapist to check for separation of the stomach muscles (diastasis recti), which needs targeted exercise rather than skin treatment.

Loose skin after Mounjaro, Wegovy or Ozempic

Weight-loss injections often lead to faster weight loss than diet alone, so the skin has less time to adapt. Many people notice it on the face first, with hollower cheeks and softer skin along the jaw, and then on the stomach, arms and thighs. The same prevention steps apply: a steady pace agreed with your prescriber, enough protein, strength training and skin protection. Read our guide to Mounjaro face for the facial changes, and see loose skin after weight loss for how we treat the body. Omnia does not prescribe weight-loss medication.

When is surgery the better option?

  • Skin hangs in folds rather than feeling soft or crepey
  • You get rashes, soreness or infections in skin folds
  • Excess skin gets in the way of exercise, walking or clothing
  • You want a dramatic change in one step and accept scarring and recovery time

If any of these apply, speak to your GP or a plastic surgeon. Our practitioners will also tell you honestly at your assessment if a non-surgical treatment is unlikely to give you the result you want.

How LPG Endermologie helps loose skin

At Omnia we use the LPG Cellu M6 Infinity, the latest Endermologie system. Its treatment head lifts and rolls the skin to stimulate the cells that make collagen and elastin, which firms and smooths the skin over a course of sessions. In a study published by LPG Systems (BeScored, 2024), 92% of participants reported firmer skin and the average waist measurement fell by 5.8 cm after 12 sessions. These are manufacturer figures and results vary from person to person.

A typical plan is one or two sessions a week for the first few weeks, then weekly, with a review along the way and occasional maintenance sessions afterwards. Sessions feel like a deep, rolling massage and you can go straight back to your day. Frequently asked questions

Can you tighten loose skin without surgery?

Yes, if the looseness is mild to moderate. Strength training, collagen-stimulating treatments such as LPG Endermologie, and a good skincare routine can all firm the skin. Large overhanging folds usually need surgery.

How long does it take for loose skin to tighten?

Skin keeps adapting for many months after weight loss. With treatment, most people notice firmer, smoother skin after six to ten LPG sessions, and results build over a full course.

Does drinking water help loose skin?

Staying hydrated helps skin look smoother and plumper, but water alone will not tighten skin that has been stretched.

Does exercise tighten loose skin?

Exercise does not shrink skin, but strength training builds muscle underneath, which fills out loose areas and improves shape.

How can I avoid loose skin after weight loss?

Lose weight at a steady pace, eat enough protein, do strength training, drink enough water, use sunscreen and avoid smoking. Starting collagen-stimulating treatment while you are losing weight can also help.
