What the test measures

The thyroid sets the pace of your metabolism, energy, weight, temperature and mood. This panel measures TSH, the signal from the brain that tells the thyroid how hard to work, the thyroid hormones free T4 and free T3, and two antibodies, anti-TPO and anti-thyroglobulin, which show whether an autoimmune cause such as Hashimoto's or Graves' disease is likely.

What your result means

A high TSH with low free T4 usually points to an underactive thyroid, and a low TSH with raised free T4 or T3 to an overactive one. A mildly raised TSH with normal T4 is called subclinical hypothyroidism and is often rechecked before any decision. Raised antibodies suggest an autoimmune thyroid condition, even when hormone levels are still normal. Your practitioner explains your pattern and refers you to your GP where treatment may be needed.

Preparing for your test

No fasting is needed. If you take levothyroxine, have the test before your daily dose and tell us when you last took it. Stop high-dose biotin supplements for two days beforehand, as biotin can interfere with thyroid tests.
