What the test measures

Bilirubin is a yellow pigment made when old red blood cells are broken down. It travels to the liver, where it is changed into a water-soluble form called conjugated or direct bilirubin, which then passes into bile and out of the body. Understanding your result

Your report shows your direct bilirubin against the laboratory reference range. A raised direct bilirubin can point to the liver not clearing bilirubin properly, or to a blockage or narrowing in the bile ducts, for example from gallstones. A normal direct bilirubin with a raised total bilirubin often fits with Gilbert syndrome or increased red cell breakdown.

Bilirubin is always read with other liver tests such as ALT, ALP and GGT. A result outside the range is a prompt for discussion with your practitioner, not a diagnosis. New jaundice, especially with abdominal pain or fever, should be assessed by a GP or urgent care promptly.

Preparing for your test

Fasting is not usually required, although a short fast of around 8 hours can make results more consistent if you are being checked for Gilbert syndrome, as fasting itself raises bilirubin in that condition.
