An oestradiol blood test measures the main form of oestrogen in the body. It is booked to look into cycle changes, menopause symptoms, fertility questions, low libido or low energy in both women and men.
Oestradiol, sometimes written E2, is the most active form of oestrogen. In women it is made mainly by the ovaries and rises and falls through the menstrual cycle, building the lining of the womb, supporting ovulation and helping protect bone. After the menopause, levels fall to a much lower, steadier level. Men also make oestradiol, mostly by converting testosterone, and it supports bone health, libido and sperm production.
Oestradiol is often read alongside FSH, LH and progesterone in women, or alongside testosterone and SHBG in men, to understand the whole hormone pattern rather than one number.

Oestradiol is read against your age, sex, cycle day and any hormone treatment. In women, a low result early in the cycle can be normal, while a persistently low level with a raised FSH can be seen around the menopause or with reduced ovarian function. Low levels can also follow very low body weight or intense training. Raised levels can occur around ovulation, with ovarian cysts, some medicines and, in men, excess body fat, liver conditions or testosterone treatment.
Your report shows the laboratory reference range for each cycle phase. A result outside the range is a prompt for discussion with your practitioner, not a diagnosis.
You do not need to fast. If you have a regular cycle and are checking ovarian function, book on day 2 to 5, counting the first day of bleeding as day 1; to check whether you have ovulated, progesterone rather than oestradiol is used later in the cycle. Men are usually tested in the morning. Tell us about the pill, HRT, fertility drugs or testosterone, and pause biotin supplements for at least two days, as biotin can affect hormone assays.

Not by itself. Levels fluctuate widely during the perimenopause, and for women over 45 the NHS usually recognises the menopause from symptoms. Testing is more helpful when you are younger or your symptoms are unclear.
Men need some oestradiol for bone and sexual health. Levels can rise with testosterone therapy or excess body fat, which may cause breast tenderness or changes in mood.
