What the test measures

L-selectin is an adhesion molecule found on the surface of most white blood cells, or leucocytes, which is where the L in its name comes from. It allows white cells to roll along and briefly grip the inner wall of blood vessels, an early step in leaving the bloodstream to reach lymph nodes or sites of inflammation. When white cells are activated, L-selectin is shed and a soluble form circulates in the blood.

Soluble L-selectin has been studied in relation to inflammation, immune activation and the health of the blood vessel lining. It is an emerging, specialist marker. It is not part of routine NHS testing and has limited use as a stand-alone screen, so it is best interpreted with established markers such as CRP, a full blood count and cholesterol.

Understanding your result

Your report shows your L-selectin value and the laboratory reference range. Levels can shift with infection, inflammation and changes in white cell activity, and research has reported both higher and lower values in different conditions. This means a single result has no simple interpretation.

Preparing for your test

No fasting is needed for L-selectin on its own. Please try to test when you have been well for at least two weeks without colds or infections, since these change white cell activity. Tell us about steroid or immune-modifying medicines you take. If you are adding cholesterol or glucose to the same appointment, we will advise on fasting when you book.
