What the test measures

Hormones work as a network, so symptoms such as tiredness, irregular periods, weight change, low mood or reduced libido can have more than one cause. This panel looks at several hormone systems together. The thyroid section includes TSH, free T4 and free T3, plus thyroid peroxidase and thyroglobulin antibodies, which can show autoimmune thyroid disease.

Reproductive hormones include FSH, LH, oestradiol, progesterone, prolactin, testosterone, sex hormone binding globulin and the free androgen index. Anti-Müllerian hormone (AMH) estimates the number of eggs remaining in the ovaries. Cortisol and DHEA-S reflect adrenal activity, and insulin and HbA1c show how the body manages blood sugar, which is closely linked with conditions such as polycystic ovary syndrome.

Understanding your result

Your report shows the laboratory reference range for each marker, many of which vary with age and cycle stage. For example, a raised TSH with low free T4 suggests an underactive thyroid, while raised testosterone and LH with higher insulin can fit a polycystic ovary syndrome pattern. A lower AMH suggests fewer remaining eggs, but it does not predict whether you can conceive naturally.

High prolactin can be caused by stress, some medicines or a pituitary issue. A result outside the range is a prompt for discussion, not a diagnosis, and your practitioner will help you decide on next steps with your GP if needed.

Preparing for your test

Book a morning appointment, ideally before 10am, as cortisol and testosterone are highest early in the day. If you have periods, the best time for FSH, LH and oestradiol is day 2 to 5 of your cycle, while progesterone is most informative about seven days before your next period, and we will help you choose the timing. Fasting is not essential, though a light breakfast helps the insulin result. Stop high-dose biotin for 48 hours, and tell us about hormonal contraception or HRT, which affect several results.
